GENERAL INCIDENT CATEGORY:
PHYSICAL INJURY (19)


GENERAL INCIDENT CATEGORY: ATTEMPTED SUICIDE (19)

DEFINITION: An act during which an individual attempts to terminate one's own life by inflicting life-threatening injury upon oneself or by placing oneself in a life-threatening situation, (e.g., by hanging, setting oneself on fire, ingesting poisonous chemicals or drugs, inflicting lacerations requiring surgical intervention, etc.).

REPORTABLE INCIDENT CATEGORIES:

01. Inmate Attempted Suicide - the attempted suicide is by an inmate.

COMMISSION OF CORRECTION REPORTING REQUIREMENTS:

1. All inmate attempted suicides, excluding those occurring in forensic mental health units operated by the New York State Office of Mental Health or the City of New York and which are located outside the confines of a county jail or county penitentiary.

GENERAL INCIDENT CATEGORY: SELF-INFLICTED INJURY (20)

DEFINITION: An act by which an individual intentionally injures oneself which is not considered life-threatening and does not represent a highly lethal attempt to terminate one's life.

REPORTABLE INCIDENT CATEGORIES:

01. Inmate Self-Inflicted Injury - the injury is inflicted by an inmate upon him or herself without direct involvement by another individual.

COMMISSION OF CORRECTION REPORTING REQUIREMENTS:

1. All self-inflicted injuries requiring one or more inmates to be admitted to a hospital or facility infirmary as an inpatient.

2. All self-inflicted injuries requiring one or more inmates to be treated by a physician at a hospital emergency room or facility infirmary, excluding first- aid treatment and non-prescription drugs.

3. All self-inflicted injuries which would require the facility physician or physician's assistant (P.A.) or facility nurse-practitioner (N.P.) to treat one or more inmates, personnel or visitors at the facility following the incident, excluding first-aid treatment and non-prescription drugs.

4. All self-inflicted injuries in which an inmate refuses to consume food and fluids for a continuous period of 48 hours.

5. All self-inflicted injuries in which an inmate refuses to consume food for a prolonged continuous period resulting in medical treatment.

NOTE: Treatment means any specific procedure used for the cure or improvement of a disease or pathological condition (e.g., stitches, medicine, sterile dressings). Examination, evaluation (X-rays, blood test, etc.) and diagnosis do not constitute treatment.


GENERAL INCIDENT CATEGORY: ACCIDENTAL INJURY (21)

DEFINITION: An injury resulting from an unexpected occurrence with no indication of direct intentional involvement of the victim or any other individual.

REPORTABLE INCIDENT CATEGORIES:

01. Inmate Accidental Injury - one or more inmates is injured.

02. Personnel Accidental Injury - one or more personnel is injured during the performance of official facility related duties.

COMMISSION OF CORRECTION REPORTING REQUIREMENTS:

1. All accidental injuries requiring one or more inmates or personnel to be admitted to a hospital or facility infirmary as an inpatient.
